*Community Resource Navigator, Healthy Hearts SF*



Public Health Foundation Enterprises (PHFE) invites applications for
full-time positions for Healthy Hearts SF Community Resource Navigators.
These positions are approximately 40 hours per week. Racial and Ethnic
Approaches to Community Health (REACH) –Healthy Hearts SF (HHSF) is a
program of the Community Health Equity and Promotion Branch of the San
Francisco Department of Public Health.



Under the supervision of the Program Director, the Healthy Hearts SF
Community Resource Navigator is responsible for working with SF Health
Network patients; linking patients to community prevention resources;
follow-up to ensure patients are reaching their health related goals;
working with the HHSF team and assisting with clinical to community
linkages as well as other duties as assigned. These positions will be
assigned to work at San Francisco General Hospital – General Medical Clinic
(GMC).



This position is 40 hours per week. Employment is provided by PHFE. This is
a one year, grant funded position. (September 2015 through September 2016).
